I have been a burn nurse for exactly one year, and have dealt with all the regular kind of verbal abuse, sexual harassment, and non-compliance we all deal with. It used to really get under my skin and I’d take it personally, but over time I learned to let it go and not live with me rent free. Also some backstory, I am 5 months pregnant and happened to mention this in front of my pt to the tech earlier in the morning.
Today, however, I finally had the experience that threw me for a loop and I guess I want to vent but also affirm that I took the right steps in this situation.
I work on the burn unit so we get mostly burns, but sometimes general off-service med surg patient if the burn census is low. Today I got this gem of a man, early 60s, morbidly obese and can barely turn/move himself so he’s on an auto turning bariatric bed, inconvenient, here for altered mental status due to acute on chronic resp failure. He has now recovered mentally but is still a patient due to other accompanying internal medicine issues.
The night RN warned me that he calls constantly (greeeeat…), but what she didn’t tell me was that he literally yells in leu of using his call light. My first interaction of the day with him was hearing him yelling “HELP HELP HELP!” loud and panicked sounding enough that I could hear him a few doors down the hallway. I immediately turn and ran into his room to find him just lying in bed staring at me, to which I said “sir, are you okay?”
Pt: “I want you to put my socks on.”
I pause because surely this guy did not just scream into the ether for me just for socks. Me: “Sir, did you yell ‘help’?”
Pt: “Yeah I need you to help me put my socks on.”
Me: “Okay, I’d be happy to help you with that but next time use the call light. I thought you were having an emergency.”
Pt: “This is an emergency, I’m cold.”
I have to just deadpan stare for a moment before going and putting his socks on. I remind him to use call light before stepping out. The rest of the shift he calls every 5-10 minutes, not an exaggeration, using either his wonderful voice, the call light or both. Unless I am in another room, I answer within 5 minutes and I know this because the call light isn’t flashing and only starts to after ten minutes of being activated. He is always incredibly rude with each interaction, and everything he needs is so incredibly unimportant, but I maintain composure.
Flash forward several exhausting hours and I am in the room with the tech getting ready to turn and clean him after a BM, but for the 100th time, remind him to please not to yell because it disrupts other patients and we can’t tell if he’s actually having an emergency or not. He begins by complaining that the response time today has been terrible, and here’s where I’m afraid I went wrong.
Me: “Sir, I have answered you quickly for the 50 times you have called me in here today.”
Pt: “Are you acting all like this because you’re pregnant?”
At this point I was fuming. I threw away my gloves and excused myself, telling the free charge nurse that I would not be going back into the room since he had been refusing all medications and anything he needed the tech could do. For the remainder of the shift I finally had time to adequately care for the rest of my patients.
I feel like I was professional at every turn, and even when I excused myself from caring for him I made sure he was covered by the charge who agreed. I just have this looming feeling that I wasn’t good enough and that I couldn’t hack it and that I should have sucked it up and taken care of him. Not sure how to feel at this point.
TLDR: After reminding a patient not to yell help and to use the call light instead, he blamed my behavior on being pregnant.
